To ensure that we correctly acknowledge you as the donor of the transferred stock, please provide the following information:
- Donor name and contact information
- Broker's fax number or email address to receive instructions on how to transfer stock directly to the Ounce's brokerage account
- The name of the stock
- Approximate number of shares
- Date of the stock transfer
